Help! Sloe'd drinks - tasting notes

I've got some mates (2) coming down at the end of the month (they're appearing on a quiz show). What I thought I would do before we go out was to use them as guinea pigs on a range of sloe'd drinks I've made (a couple of slerry's, some gins made with and without frozen sloes, some sliders and some pataxaran).

 

I could do it myself, but I'm a bit worried about suffering from "Ikea syndrome" (that is: it's from ikea so it tastes lovely). To get around this I've had a little look for something to record tasting notes (to report back on the results of various experiments).

These are the most suitable ones I could find - what I wanted was something I could derive an average opinion from?

 

Anyone suggest anything different or better - maybe any modifications to the above?

 

EDIT: I'm interested in their (honest) opinions as neither of them has ever heard of sloe gin. Well that's a lie, one of the does know what it is only after I introduced him to it and after a bottle he was definitely a fan. Here's what he wrote in an email telling me about the date of the recording:

 

 

Can't see the pictures at work but our recording is on the XXth/XXth Sept.

Get that gin on!
