Yup although if you have convictions but nothing new to add you can just say as per previous application.

This is the public sector you're dealing with. I've had the same name and date of birth all my life but do they believe me???

The simple answer to your question is: `Yes` You do need to fill in all parts of the form regardless of the fact that many of your details have not changed over the last 5 years.

As you live in Kent you will also have to get your GP to provide a letter and send it direct to the Police at Maidstone confirm you are free from a number of medical conditions.  You may have to pay the GP for this service. Some charge, some do not. Mine charged £25 which I thought was a fair price, in the light that without this letter Kent Police will reject your application.
