What a terrible thing to happen, how must he be feeling now?

Someone didn't check everything quite as closely as he should, I should think the Film Armourer will be feeling pretty awful too.

Not the first, that’s for sure.

Incidents in the past have occurred from those placing the barrel against a temple, the percussive force coupled with any wadding or cardboard disc is enough to inflict trauma.

We in the Uk tend to think of blank firing guns as having an obstructive barrel and gas discharge port located elsewhere. Whereas these are real firearms loaded with blanks. 
Why do you think the mill has blank firing  adapter- helps to mitigate this or the accidental insertion of a live round.

Accidents happen unfortunately and we do not have enough information on this one.

Yep this is my guess. 

Dummy ammo that gets used alot and the bullet dislodges getting stuck in the barrel, firearm gets put away for a period of time. It's later bought out and loaded with blanks, not knowing there's a projectile lodged in the barrel, actor pulls the trigger and we know the rest. 

Absolutely tragic, awful for all involved.

Not quite.

Previously the S&W had been loaded with ‘dummy’ rounds - except the dummy who did so, didnt remove the primer and so the bullet was subsequently lodged in the barrel as the actor made out to shoot Lee in the scene. The revolver was not checked for any obstructions!

These ‘dummies’ were needed for a certain angle and show the cylinder with ‘rounds’ in and not an empty revolver- cinematography. 

For a later ‘take’ , the dummies were replaced with blanks which would then give the correct muzzle flash- well, we know what happened next. 
 

For authenticity they needed the flash and the look of a full cylinder. In the last few years they have been using cgi to portray this, but it looks too ‘artificial’.

We don't know who's been negligent yet - if anyone. These days sets involving weapons have trained professionals whose sole responsibility is the firearms, even down to plastic imitations. They are there to stop amateurs shouldering responsibilities they're not qualified to take. Their word should be immutable and so when someone's handed a 'cold gun' they are the ones taking the responsibility to make sure it is, not the actor who might not know anything about what the chamber and rounds should look like - or if one of the rounds looks faulty, for example. Until we know more, we can't say who's to blame.

 

The terrible coincidence is that this all happened on the set of a film based on the premise of an accidental shooting.

The gun pictured in the above link is in fact a percussion (i.e. muzzle loaded) weapon.

But you can expect reporters to ba able to get their facts right!

I read somewhere that it could have been a case of murder by proxy
