Getting written permission to shoot

I want to get written permission to shoot on land.

 

I saw a really good thread that had a form that I could hand to the land owner to complete but I can't find the thread anymore.

 

Can anyone post something like this for me to print and use?

Got to BASC's website they have a permission form template on there which I just amended slightly for my own use
