There isn't a generic form for variation, send back your FAC with the disposal notice and fill out your FAC application without the referees.

The variation it the application form, a FIR 101 in most regions.

 

You do not have to supply all the photos or references again, just fill in all the appropriate bits and send off your £26, unless it is a 1 for 1 when it should be free!

On the bit where it asks what you're applying for, remember to put down any slots you have currently that are not filled too.

 

When I did my variation (albeit not a 1-4-1) I didn't put down the empty FAC Air moderator slot that I already had, cert came back with all the new slots I'd asked for and the guns/moderators I already owned but I lost the FAC Air moderator slot. Not a big problem as I don't actually need it and fairly sure I could get it back with a phone call to the FLO but saves you a bit of hassle not to have to.
