phaedra1106
Form 201 has been changed yet again. For the medical declaration it now asks you to list all GP practices you've been registered with in the last 10 years and also adds the following question, "Are there any periods in the past 10 years when you have not been registered with a UK GP or have consulted medical practitioners other than at your GP practice?" So do we now have to list every hospital visit to say A&E, EN&T, diabetic clinic or having a boil on your bum looked as each time you will have consulted a medical practitioner outside of your GPs practice
ozzy518
I'd say visits to a and e and suchlike would be entered onto your medical records anyway. Tho I reckon I'm gonna have to declare my privately paid vasectomy!

David BASC
Its only relevant to the points listed in the notes at the back of the form. For example, just because I have to go for checks on a lesion at the back of one of my eyes at the local hospital, which does not impact on my vision at all, and was picked up at an regular eye test at Asda does not need to be reported at my renewal next year David
phaedra1106
I contacted David at BASC, "Having looked again I think its important to recognise that this only relates to conditions that must be declared ie:Relevant medical conditions which must be disclosed include, for example:Acute Stress Reaction or an acute reaction to the stress caused by a traumaSuicidal thoughts or self harmDepression or anxietyDementiaMania, bipolar disorder or a psychotic illnessA personality disorderA neurological condition: for example, Multiple Sclerosis, Parkinsons or Huntingtonsdiseases, or epilepsyAlcohol or drug related abuse"
Jay_Russell
Ludicrous. In the last ten years I often spent months in different areas of the country for work. Sometimes I registered somtimes I didn't. How on earth am I supposed to remember every doctor I visited, when I registered and when I didn't. Medical records are just that, records. It's the same which ever doctor I used.
phaedra1106
It's only for the conditions you're already required to disclose (see list above) so anything else like minor injuries at A&E etc. you don't need to declare.
