Big_Sam
Its all different for firearms and shotguns. For a shotgun certificate you need a person of good standing in the community. For a firearms ref it can be generally almost anyone who is law abiding and knows you. I use my local minister. Guns + religion, a useful combination sometimes

Big_Sam
Your general work number is fine. Its just to check that you are who you say you are. For the medical questions you MUST answer truthfully. They have the right to approach your doctor for factual information about your medical history if there is reason to need it. It doesn't mean that you will be refused a certificate, its just that they are required by law to check. You must list it, even if it was years ago. If you are no longer suffering from it, and your doctor is satisfied you are fit and well, then there should be no problems. If in doubt, phone the firearms department and ask their advice. I have always found my department to be extremely helpful.

Big_Sam
I would write an accompanying letter listing all treatment. Then write on the application form, 'see letter'
