Moderator Problem

I have a howa 1500 .223 stealth kit, it comes with a sonic 45 moderator. There were no fitting instructions but it looked fairly straight forward, it isn't the type of moderator that screws directly on to the barrel. It came with 2 plastic round fittings, one screwed into the barrel the other into the moderator reducing the diameter of the hole that goes onto the barrel.I tested it today and the moderator blew off when I took a shot, I assumed it was because it wasn't on tight enough. So I tightened it further and took another shot. It blew off again and the gases expelled blew back into my face. On inspecting the moderator the bullet seems to have clipped the end of the moderator on the way out. Now the big plastic part is stuck in the thread of the moderator and the smaller one is inside.

Does anyone have the setup? What have I done wrong (if anything)?

When you say one screwed into the barrel do you mean onto the screw cut of the barrel rather than into the barrel? I'm not being sarky just checking what actually happened...

If it's a plastic spacer that just increases the external diameter and the mod works purely on friction to stay on it sounds right weird? Did you try removing the bolt and sighting down the bore to see if the hole has an obvious obstruction from the mod? Either way it sounds cream crackered.

I have one on a 250, it has a thread in the large section of the mod that screws onto the barrel. The plastic ring screws into tbe backend of the mod. It does not/ or shouldn't be tight to tbe barrel or shouldn't be nesasary if the screw cut is square. What actually got blown off ??? The whole mod or some of the baffles ??? Never had any issues with mine apart from anable to part some of the baffles. But what I paid for it its not exactly jetz money. But its light which is why I have it.
