PhilR Posted March 22, 2015 Report Share Posted March 22, 2015 Is there a form on your local police force website? As far as I'm aware there isn't a generic form as all forces are different entities. I write a letter giving all relevant details and email it to my force, Thames Valley. I sometimes receive an automated reply, one from an FLO and sometimes nothing.
