Same from West Mercia, though does offer alternative - see photo which is the instruction to the Doctor supplied by the Police and has to be signed by me before the forms are sent to the Doctor. Gordon R
Why has the OP not said what really happened, blow by blow? These threads usually start out with half a tale and further details emerge drip by drip. They never seem to end with the full facts and the OP disappears. No sign of him / her for nearly a week. This has all the hallmarks of such a thread.

Downforce
As Dekers already said This is now going round and round Key points seems to be IF the doctors sent something they shouldn’t that is obviously wrong they should be challenged and there are data protection laws to address incorrect personal data Addressing incorrect personal data is not always the same thing as agreeing with the data provided by the doctors Once it reaches the police it’s more difficult to put “back in the box” but obviously it should be possible within the system to correct genuinely incorrect data It also seems different police areas have different procedures and protocols so understand what the process is before submitting and where possible review the data before it’s submitted from doctors to police Some of that will help others in addition to the OP and hopefully that helps the OP and parks any questions about the exact situation and circumstances which other contributors don’t necessarily have a right to know just because they posted on the internet. if the OP wants to update with an outcome that would be nice but it’s not their obligation to do so D
