I hate to say it but I've probably crossed my toes a few times when I've pointed the muzzle down quickly for some reason or other. I heard of someone who shot himself (with a .45 I believe) when he took the safety off while the gun was holstered during a practical pistol competition (when handguns were legal here). There was a bang and he said something like "I think I've just shot myself" and promptly fell over. The bullet went down inside the length of his thigh and because it was teflon coated the surgeon had to open his thigh the full length of the wound to clean it completely.
