Just note down the serial number of the gun, wether given/lent/sold, cert number of the other party involved, your cert number and the date, obviously sign it, scan it for your records and send it recorded delivery.

The law simply requires you notify certain detail within a certain time

There is no prescribed or mandatory form you have to use, but most forces have a form they provide to assist in getting the right information. If you download a form from any police site, it should be perfectly acceptable with any other force as it simply asks for the precribed information.

Essentially all you are doing is:

Identifying yourself / cert no

Stating what changes have been made

sold/lent/bought

a makers name / cailbre / type

from, cert no or rfd and details

The record should be independently confirmed by the other party, and if both tie up then you will not hear from the FLD. If there is a discrepancy or failure by one party to notify, expect an enquiry.
