My lad has just renewed his coterminous and the region actually sent him old style forms to complete for the renewal, he filled them all in and returned (and referees) only for the region themselves to refuse/return them to him saying they had to be on the new style form. To say we were ****** off is an understatement, especially as they didn't even send him the new forms but told him to download them off their site, 11 pages worth I seem to remember! Both the forms the OP is questioning are the prescribed form 201. It's just that one is editable as per e-commerce implementation and the other isn't. Surrey have added the requirement for the referees to sign the application, this is NOT required using the correct version of the 201 form, you only have to provide the referees details. nb.
