Under the new rules do you need to supply a complete medical history or just changes since the last renewal or my FAC grant about 18 months ago? TIA You need to go coterminous with the new fees coming out. Lancs renewal requires 10 years but others want to know what you were up to when you were a baby. That's how it worked, when I did my renewal, all this talk of they want 10 years/full medical history threw me. When I did my renewal, I had the proforma and the doctors notes, Kent said only send the proforma. rbrowning2 Posted January 23 Report Share Posted January 23 (edited) as above the FEOs are not able to understand medical notes, so your registered GP fills in a Police pro forma report you pay for that service and the report is send in theory directly to the police. If your registered GP will not do it then you need to use one of the GPs that have set up in business to do it. They will get you to give them consent to obtain your medical records and then complete the police pro forma and sent it to them. Which ever way you do it, do get a copy of what is sent to the police so you can check it is correct.
