Dougy Posted January 6, 2019 Report Share Posted January 6, 2019 (edited) First, i cant say exactly what to have signed by whom, but I can advise that you read the guidance notes that come with the application form. If they re not with the form or mislaid then the a look on BASC's web site. It's only once in 5 years you need to do this so it's not too much of a pain.
