nellywinner
When you apply for Firearms or SGC there is a Medical Information Proforma sheet you download and print from your County Firearms Licensing Department. Then drop this off to your doctor to fill in, they may or may not charge you for this service. It took 6 weeks to get my form back filled in at a cost of £55, then attach it by pdf copy to your application. Some doctors send direct to Police by secure email. Not sure about "back to birth" bit, but what they require is on the Proforma they supply. 15 minutes ago, nellywinner said: When you apply for Firearms or SGC there is a Medical Information Proforma sheet you download and print from your County Firearms Licensing Department. You just need their medical proforma, print it off, fill in your bit name etc. and hand it to the GP. They will do the rest. Providing your GP actually does them, mine does, but it took over 3 months. Edited February 24, 2023 by Newbie to this

You need to go through the renewal process on the police website, after clicking a few boxes you should arrive at the medical proforma, you can then download it. or try this link?...https://www.northwales.police.uk/SysSiteAssets/media/downloads/central/services/apply/firearms/legislation-change-forms-2021---in-use/gp-proforma/gp-proforma.pdf

Whoever told you that is mistaken.
snow white
The lad on the farm where we live as said they sent his renewal back telling him to get back to birth records. My doctor as already filled my proforma in for me no charge just in process of filling the rest of paperwork in then the long wait again. I don’t now about you lads I think its someone in the office not knowing what they are talking about, there is nowhere in the application forms or the medical proforma that asks for your medical records from birth, what they may be referring to is where they ask the doctor if they have full medical record as highlighted below in red MEDICAL INFORMATION: To be completed by doctor**A doctor with a full, specialist or GP (rather than provisional) GMC registration and a licence to practise. Please check the patient's medical record for any history of the following and tick those that apply. Where any apply, please add further details overleaf which can be limited to a statement of fact and not an opinion. Have you had access to the patient’s full medical record to complete this report? Yes No Is the medical record continuous? Yes No Have you placed a ‘firearm application made’ flag on the patient record? Yes No DATE RECORDS BEGIN: ………………..DATE OF LAST CONSULTATION:

I don’t now about you lads There is a missunderstanding going on here .... You do not have to supply your medical records. I think @old'unhas hit the nail on the head. Your 'lad on the farm' hasn't fully understood what has been asked and niether has the FLO at NWP. Anyway i'm glad you're all sorted @snow white

welsh1
The police don't know how to decipher your medical records, it's up to your doctor to supply the information requested with the information they have on you, and the doctor will then let the firearms dept know their assesment of your health and welbeing. The first 3 years of my life were spent in germany, there is limited medical records on me in this period, from 1983 to 1991 all my medical records are missing, well i say missing they are in a big hole in the desert in the gulf along with many others, despite that a letter from my doctor saying he thinks i am not totally insane and he would let me have a firearm. This is Dyfed Powys.

London Best
I know some of my records are missing, because in 1979 I was very ill with hepatitis for a long time. There is no mention of it in my medical records.
