When you apply for Firearms or SGC there is a Medical Information Proforma sheet you download and print from your County Firearms Licensing Department. Then drop this off to your doctor to fill in, they may or may not charge you for this service. It took 6 weeks to get my form back filled in at a cost of £55, then attach it by pdf copy to your application. Some doctors send direct to Police by secure email.

Not sure about "back to birth" bit, but what they require is on the Proforma they supply.

Mine cost me £90 and took 7 days.

You just need their medical proforma, print it off, fill in your bit name etc. and hand it to the GP. They will do the rest. Providing your GP actually does them, mine does, but it took over 3 months.

You need to go through the renewal process on the police website, after clicking a few boxes you should arrive at the medical proforma, you can then download it.

That's inncorrect, you do NOT need to supply your medical records at all.
Whoever told you that is mistaken.

I think its someone in the office not knowing what they are talking about, there is nowhere in the application forms or the medical proforma that asks for your medical records from birth, what they may be referring to is where they ask the doctor if they have full medical record as highlighted below in red

There is a missunderstanding going on here ....
You do not have to supply your medical records.

I think @old'unhas hit the nail on the head.

Your 'lad on the farm' hasn't fully understood what has been asked and niether has the FLO at NWP.
Anyway i'm glad you're all sorted @snow white

The police don't know how to decipher your medical records, it's up to your doctor to supply the information requested with the information they have on you, and the doctor will then let the firearms dept know their assesment of your health and welbeing.

The first 3 years of my life were spent in germany, there is limited medical records on me in this period, from 1983 to 1991 all my medical records are missing, well i say missing they are in a big hole in the desert in the gulf along with many others, despite that a letter from my doctor saying he thinks i am not totally insane and he would let me have a firearm.
